2021 VeggieRx for Veterans

Veterans that have diagnosis of pre-diabetes, diabetes, heart disease, or obesity and have an insecure food source VeggieRx could be a great program for them to enroll in. Since the pick-up locations are at a medical facility and this is part of treatment, they should be able to use the free veteran transportation through CET.
 
 
Thanks to funding from the Oregon Department of Veterans Affairs, HDFFA will be able to serve 75 Veterans - 3x our capacity in 2020!

ELIGIBILITY:

  • Must be a veteran
  • Must have diagnosis of pre-diabetes, diabetes, heart disease, or obesity
  • Must be food insecure
     

PROGRAMMING START DATES:

Prineville: Thursdays 12-2 starting June 17th at St. Charles Family
Bend: Wednesdays 3-5 starting June 30th at Bend's Community Based Outpatient Clinic (CBOC)
 

ENROLLMENT PROCESS:

  • Individuals can access the eligibility screen by texting "VEGGIE” to the phone number 797979
  • By link: www.surveymonkey.com/r/Apply-VeggieRxVet
  • Or CHW’s/staff can save link and help patients fill out the materials
